I have really bad underarm perspiration?

Medical antiperspirants don't work. No deodorants work. Most antiperspirants give me rashes. The only thing they seem to do is mask the smell from the sweat. Is there anything i can do. I go through about 4-5 shirts a day. reapplying the deodorant doesn't work either.
